CRITERIA FOR REQUESTING A BASIC SET OF NEUROSURGICAL INSTRUMENTS, AND/OR A MICROSCOPE OR BIPOLAR COAGULATION SET
Any person requesting a Basic Set of Neurosurgical Instruments, a Microscope and/or a Bipolar Coagulation Set needs to fulfill the following requirements:
- Be a qualified and practicing neurosurgeon.
- Be a member of a WFNS member society, which is in good standing with WFNS and not in arrears of payment of membership fees.
- Have the approval of the President and/or Secretary of the Society of which he/she is a member.
- Fill in, date and sign the WFNS Application Form and return it to the WFNS Central Office by email or fax.
- Purchase the Set, Microscope and/or Bipolar Coagulation Set him/herself or jointly with one or more other neurosurgeons.
6. In the case that he/she cannot purchase the Set, Microscope and/or Bipolar Coagulation Set, seek a donation from the Society of which he/she is a member or try the utmost to find a sponsor.
7. Confirm that the Set, Microscope and/or Bipolar Coagulation Set will not be used in a Private Hospital or Clinic where high fees are charged for treating patients.
8. Confirm that the Set, Microscope and/or Bipolar Coagulation are not for his/her own personal use.
9. Request only 1 Set, Microscope and/or Bipolar Coagulation Set per Hospital, unless purchasing it him/herself or through a sponsor.
10. Agree to the possibility of being listed on the WFNS website for having received a Basic Set of Neurosurgical Instruments, a Microscope and/or Bipolar Coagulation Set.
11. Be willing to automatically provide a report, from time to time, on the performance, use and efficacy of the Basic Set of NS Instruments, the Microscope and/or the Bipolar Coagulation Set.
